Stroke

Stroke bagi kebanyakan orang merupakan suatu penyakit yang sulit disembuhkan dan akan mengakibatkan kelumpuhan dibagian tubuh tertentu, dalam hal ini saya akan sampaikan sedikit informasi mengenai stroke.
Stroke merupakan penyakit gangguan fungsional otak berupa kelumpuhan syaraf/defisit neurologik akibat gangguan aliran darah pada salah satu bagian otak.
Secara sederhana stroke didefinisikan sebagai penyakit otak akibat terhentinya suplai darah ke otak karena sumbatan atau pendarahan, dengan gejala lemas/lumpuh sesaat, atau gejala berat sampai hilangnya kesadaran, dan kematian.Stroke berupa iskemik maupun perdarahan (hemoragik).
Pada stroke iskemik, aliran darah ke otak terhenti karena aterosklerotik atau bekuan darah, melalui proses aterosklerosis.
Pada stroke pendarahan (hemoragik), pembuluh darah pecah sehingga aliran darah menjadi tidak normal dan darah yang keluar merembes masuk ke dalam suatu daerah di otak dan merusaknya.
Stroke akut,baik yang iskemik maupun hemoragik, merupakan kedaruratan medis yang memerlukan penanganan segera karena dapat menimbulkan kecacatan permanen atau kematian.
Menurut WHO, stroke adalah tanda-tanda klinis mengenai gangguan fungsi serebral secara fokal ataupun global, yang berkembang dengan cepat, dengan gejala yang berlangsung selama 24 jam atau lebih, atau mengarah ke kematian tanpa penyebab yang kelihatan, selain tanda-tanda yang berkenaan dengan aliran darah di otak. Ada 3 (tiga) pilihan bagi penderita Stroke bisa sembuh sempurna, sembuh dengan cacat, atau berakibat kematian

Back rub For Tirah Baring

Tirah baring diartikan sebagai tinggal ditempat tidur untuk jangka waktu yang lama dan diharuskan. Kata "istirahat" berkenaan dengan hal ini agak kurang tepat karena kita selalu berfikiran bahwa ini diartikan dengan istirahat malam yang baik. Pada tirah baring sebenarnya bukan sesuatu yang dilakukan dengan sukarela.Individu tak secara wajar berfungsi diluar tempat tidur ini sebagai akibat dari berbagai gangguan fungsi ( gerak, bernafas, pengendalian syaraf ). Ini sebagi akibat dari penyakit (panas tinggi), kelemahan (lumpuh ).

Tirah baring sempurna ( bed rest total): - Penderita ditidurkan dalam keadaan terlentang dengan posisi kepaIa diangkat (diberi aIas bantal) kurang dari 60° (1), Hal ini akan mengurangi tekanan darah pada pembuluh darah iris serta memudahkan kita meng-evaluasi jumlah perdarahannya.

Dekubitus adalah salah satu bahaya yang terbesar pada tirah baring. Dalam sehari-hari masyarakat menyebutkan sebagai "akibat tidur". Suatu luka dekubitus disebutkan oleh karena ada tekanan pada kulit.. Tak lama kemudian akan terlihat pada tempat-tempat yang mendapatkan tekanan, warna-warna kulit yang memutih.Jika penekanan ini hanya berlangsung untuk waktu sementara, maka tidak ada akibat-akibat yang merugikan bagi aliran darah. Pada penekanan yang berlangsung waktu yang lama, maka timbul masalah dalam peredaran zat-zat makanan dan zat asam yang harus disalurkan pada bagian bagian kulit. Yang mengalami penekan tadi, jaringan –jaringan yang tak mendapat cukup makanan dan zat asam tadi perlahan akan mati, dari sinilah kemudian timbul luka-luka dekubitus (Maklebust, 1991).
Back rub ( gosokan punggung ) adalah suatu bentuk massage pada punggung yang mempunyai tujuan untuk merelaksasi dan mengurangi tekanan . Gosokan dari prosedur ini akan menghasilkan panas pada permukaan kulit. Hal ini menyebabkan dilatasi pembuluh-pembuluh darah sehingga akan meningkatkan supplay darah kedaerah tersebut. Karena jaringan sering tertekan pada pasien yang istirahat di tempat tidur dan otot-otot biasanya relaksasi, stimulasi penting agar jaringan mendapatkan nutrisi dan oksigen.
Berdasarkan pengalaman peneliti selama melaksanakan tugas di Rumah Sakit dan hasil dari beberapa intervensi keperawatan yang dilakukan oleh perawat hanya melakukan motivasi dorongan kepada keluarga pasien untuk melakukan alih baring setiap 2 jam kearah kanan dan 2 jam kearah kiri. Tanpa melihat sejauh mana efektifitas keberhasilan dari alih baring tersebut, sementara pasien tetap terjadi dekubitus. Melihat kenyatan tersebut diatas dan sekaligus untuk mengetahui seberapa jauh efektifitas massage punggung, penelitian tentang efektifitas massage punggung terhadap upaya pencegahan dekubitus pada pasien yang bed rest total. 50 % - 74 % dilaporkan untuk sering dipantau atau ditangani dengan baik.
Luka dibubitus adalah nekrosis seluler yang cenderung terjadi akibat kompresi berkepanjangan pada jaringan lunak antara tonjolan tulang dan permukaan yang padat , paling umum disebabkan karena imobilisasi. Faktor ektrinsik yang mengeluarkan kekuatan mekanisme yang pada jaringan lunak termasuk tekanan, gesekan, friksi, dan maserasi. Faktor Intrinsik yang menentukan kerentanan kerusakan jaringan mencakup malnutrisi, anemia, kehilangan sensasi, kerusakan mobilitas, usia lanjut, penurunan status mental, inkontinensia, dan infeksi. Faktor ektrinsik dan intrinsic berinteraksi untuk membentuk iskemia dan nekrosis jaringan lunak pada individu yang rentan. 80 % luka dicubitus yang sembuh terjadi lagi, banyak diantaranya karena ketidakberhasilan mempertahankan regimen pencegahan ulkus (Maklebust, 1991).

Potret Kehidupan

Allah SWT menciptakan manusia berpasangan dalam suatu ikatan perkawinan. Dari perkawinan tersebut terbentuk sebuah keluarga yang terdiri dari ayah, ibu, dan anak-anak. Dalam kelanjutannya orang tua bertanggung jawab penuh terhadap kelangsungan hidup keluarga, memelihara dan mendidiknya dengan baik, memberi kasih sayang kepada mereka, sebagai tanggung jawab kepada Allah SWT terhadap amanah yang telah diberikan-Nya.
Utamanya tugas seorang ayahlah mencari nafkah untuk menghidupi keluarganya, ibu mengurus rumah tangga, mendidik dan merawat anak-anaknya dirumah. Selain anak juga mendapat pendidikan formal disekolah. Namun pada kenyataannya banyak dijumpai anak-anak yang tidak bisa merasakan hidup layak sebagaimana mestinya. Mereka hidup terlantar tanpa kasih sayang dari orang tua/keluarga, tidak dapat mengeyam pendidikan dibangku sekolah, tidak dapat menikmati hidup sebagai anak-anak yang penuh suka cita, tumbuh dan berkembang sesuai harapan.
Kondisi demikian terjadi karena mereka tidak mempunyai orang tua/keluarga lagi atau karena orang tua telah bercerai. Ironisnya bila mereka masih mempunyai orang tua tetapi malah ditelantarkan, dipaksa bekerja mencari nafkah yang sebenarnya belum menjadi tanggung jawab mereka. Apalagi bila terjadi kekerasan dalam rumah tangga. Adanya ketidakseimbangan ekonomi yang semakin lebar menjadi salah satu factor utama penyebab berbagai macam tindak pidana. Dan pihak-pihak yang sering dirugikan akibat hal tersebut adalah perempuan dan anak-anak. Apalagi budaya kita yang sering menempatkan perempuan dan anak-anak sebagai pihak yang lemah. Perempuan termasuk anak perempuan, sering menjadi korban kekerasan karena seksualitasnya sebagai perempuan, mereka sangat rentan terhadap kekerasan yang dilakukan oleh orang-orang disekitarnya, di ruang public, tempat bekerja, bahkan dirumahya sendiri. Prilaku negatif dan menyakiti yang dialami oleh anggota keluarga akan membentuk jiwa anak menjadi tidak baik, anak menjadi stress, pemarah, tidak peduli dengan lingkungan.
Dalam benak mereka yang ada hanyalah kebencian, permusuhan, kekerasan dan tiada kasih sayang.
Sering dijumpai diberbagai tempat seperti di perempatan jalan, lampu merah, angkuatan-angkuatan umum, atau ditempat-tempat sekitar rumah ibadah, anak-anak yang berjualan koran, mengamen bahkan meminta-minta. Tidak sedikit ada anak yang nekat melakukan tindakan kriminal seperti menjadi pencopet dsb, lebih menyedihkan lagi bila anak terjerumus kedalam dunia hitam prostitusi. Mereka tidak lagi memikirkan masa depan, norma-norma dimasyarakat, baik buruknya tindakan mereka. Yang ada dalam pikiran mereka hanyalah bagaimana mereka bisa mendapatkan uang demi sesuap nasi untuk menyambung hidup mereka.

THROUGH THE ACT OF DOMESTIC VIOLENCE:

Domestic Violence (domestic violence) has become a common agenda in recent decades. The evidence suggests that domestic violence affects large enough for a woman as victim.

As with rape, beating of wife became public and became a very difficult situation for women in every nation, caste, class, religion or region.

At the international level, violence against women was viewed as a frame of crime against women's rights and fundamental freedoms and the destruction and removal of their freedom against the rights attached to him. This becomes a challenge in achieving equality, development and peace are recognized in the Nairobi Forward-looking Strategic for the Advancement of Women [6], which recommends a set of measures to combat violence against women. The recommendations are charged to the Government as the legal and moral obligation to eliminate domestic violence through a combination of various means.

Domestic violence is a problem that has been rooted very deep and occurs in all countries of the world. In this case, the international community has created an effective and legal standards specifically address domestic violence. Of battered women, for example, has been included in the international and regional human rights conventions that are legally binding on countries that have ratified it. International human rights documents include, the Universal Declaration of Human Rights ("UDHR"), the International Covenant on Civil and Political Rights ("ICCPR"), and the International Covenant on Economic, Social and Cultural Rights ("ICESCR") is the standard General on Human Rights, where the victims of domestic violence to sue their respective countries. Various pertistiwa domestic violence has shown that the state has failed to adequately address their grievances. Then the state may apply sanctions if the country is a member of the international instruments as mentioned earlier. The same can also be done under the Convention on the Elimination of All Forms of Discrimination Against Women ("CEDAW") together with its Optional Protocol, and also through the Convention Against Torture and Other Cruel, Inhuman, or Degrading Treatment or Punishment ("CAT") . Likewise, a regional instrument to provide protection to women victims.

The European Convention for the Protection of Human Rights and Fundamental Freedoms ("ECHR"), the American Convention on Human Rights ("ACHR"), together with the Inter-American Convention on the Prevention, Punishment and Eradication of Violence Against Women (" Inter-American Convention on Violence Against Women "), and the African Charter on Human and Peoples' Rights (" African Charter ") is the main regional human rights document that can be used as the basis for victims of domestic violence.

Negative effects of domestic violence also varied and are not only family relations, but also against members of the family is in it. In the case of serious physical and psychological injuries directly suffered by female victims, continuing and endemic nature of domestic violence limits women's opportunities to gain equal rights in law, social, political and economic in the midst of society. Regardless of the victimization of women, domestic violence also resulted in a family relationship breakdown and children who then can become a source of social problems.

Violence among those who have a close relationship, as already described above is one major problem in Indonesia, as well as around the world including India. One common approach to overcome this problem to look to the law. Thus, women's advocates both in Indonesia and in India to be with doing repairs sought legislative and policy measures that criminalize domestic violence.

This study memeberikan focus on the issues of protection of women through the Domestic Violence Act in developing countries, namely Indonesia and India. Description and comparative analysis presented for the legal solution of the problem. The study found that people in a country provides an enormous influence in the manufacturing process of Domestic Violence Act, in which between India and Indonesia have a different characteristic to defining domestic violence. For example, India named her as the Act on Protection of Women from Domestic Violence Act, 2005, while Indonesia mentioned it to the Elimination of Violence in Household Act, 2004.

Further, India knows "dowry" and "sati" as a domestic violence-specific, while Indonesia is not about both. However, Indonesia has a very broad definition to overcome all forms of acts of domestic violence. Finally, this study tries to provide some input in order to address one of this violence is significant.
